We regret to inform you that we will not be part of the Providence Connected Care product in 2016 and will be considered out-of-network. In order for you to maintain in-network benefits you will unfortunately need to transfer care to a contracted Providence Connected Care provider during your open enrollment period in October 2015.
If you choose to be seen in our office your visits will be applied to your out-of-network benefits. Since we cannot be assigned as your PCP while being out-of-network providers, we will be unable to refer you to specialists if needed. You can expect a higher copay and lower reimbursement rate if you choose to stay under our care.
